Permission to Participate Form
Linked here Permission to Participate Form is the Official Form used by Visitation School, the Kansas City Parochial League and the Diocese of Kansas City-St Joseph for all the Visitation sponsored sports.
To ease administration of these forms, you may complete one form for each athlete and simply circle all the sports your athlete may participate in for the entire 2008-2009 school year.
Please use the form linked above which includes a line for the e-mail address or addresses you wish to use for communication from your player's coach and manager.

All-Sports Signup - Details, Fees and Instructions
Okay, if you missed the All Sports Sign-up held Aug 6, you need to get your forms and payments in as soon as practical directly to our Athletic Director: J.L. Roach. 414 W 63rd Street 816-361-8652.
Please take the initiative to turn in your forms, it will ease your coaches burden of chasing you down to get your Permission Forms and Payments.

Background Check Authorization for Volunteers
The policies set forth from the National Conference of Catholic Bishops requires that all volunteers and employees who have regular contact with children complete a background check and attend a workshop entitled "Protecting God's Children". This includes parents who help in the cafeteria, assist in the classrooms, and coach sports.

End of Season Evaluation Form
Click here End of Season Evaluation Form to download a form you can use to provide comments about your child's participation experience in any Visitation sport. Your feedback is very important in making the Visitation sports programs as successful as possible.
Be assured these comments (both complimnentary and critical) are well read and taken very seriously by the Sports Coordinators, the Athletic Director and the Visitation Athletic Committee.
